Food log template that I created

This is from 2014, and the OP hasn't been active on the forum in a couple of years. I don't know what exactly this document was, but you can keep a food log here on the forum in the Whole30 Log section, you can keep a written journal, make a Word document on your computer, keep a list in Evernote, you can take pictures of your meals and post them on Instagram or even Pinterest. You can keep track of just the food, or also track how you're feeling emotionally or physically, including tracking specific physical symptoms if you want to.

Make each of your meals look like the meal template, and you'll be fine. If you come up with a format you think other people might find helpful, you can certainly post that in the forum to share.
