Meal planning: protein + fat + veggies (carb dense + not carb dense) +.... Fruit?

You should limit fruit to no more than two servings a day. Most people do best with at least one serving of carb dense vegetables a day, and some need more. It's fine to have both at the same meal, but do pay attention to how you feel afterwards -- people who have blood sugar issues may find that having both makes them feel less good, or if they measure their blood sugar levels may find it causes issues, and that they prefer not having both in the same meal.
