It's hard to say without more information. Could you list a day or two of typical meals, including approximate portion sizes as they relate to the meal template? 

Are you salting your food? Eating at least a serving of starchy vegetables each day? Drinking plenty of water? Adding fat to every meal in addition to what you cook with? Do your meals keep you satisfied 4-5 hours at a time?
