When to start?

It's really up to you. Know that if your surgery involves anesthesia, they may make you eat something afterward to ensure you can keep food down, and whatever they give you is likely not to be whole30 compliant. You would either need to talk to your doctor beforehand and find out what you can bring that is compliant, or you'd be restarting after that anyway.

Do plan ahead and have some food that won't require more than heating up on hand for the days after your surgery. If it's something where getting up and around is going to be difficult, possibly even freeze some meals to get you through as long as your recovery is expected to take.

I'd also plan to have some broth or soup on hand afterward so that even if you don't feel like eating, you can get some kind of nutrients in. Plus, broth is thought to be good for healing. 

It is definitely possible to start this before or after surgery, it's really up to you to decide what makes the most sense for you.
